## TL;DR

A research blood test that looks for six digestive-system cancers at once and predicts which organ the signal comes from.

## Summary

Kandimalla et al. (Clinical Cancer Research 2021) validated the panel in 300 cell-free DNA specimens: a pan-gastrointestinal panel detected all six cancers (colorectal, hepatocellular, oesophageal squamous and adenocarcinoma, gastric, pancreatic) with an area under the curve of 0.88, and the EpiPanGI Dx tissue-of-origin panel predicted the organ with accuracy of 0.85 to 0.95 for most. It is an academic assay without regulatory status.

## Fields

- Kind: Treatment
- Status: emerging
- Last checked: 2026-10-29
- Tags: test; mced; evidence:insufficient
- Brand: EpiPanGI Dx
- Modality: Gastrointestinal multi-cancer blood test, academic research assay (cell-free DNA methylation)
- Mechanism: Targeted bisulphite sequencing of 67,832 methylated regions chosen from tumour tissue, with panels to detect any digestive cancer and to predict which organ it is in.

## Notes

- Evidence grade insufficient for the claim that taking this test reduces late-stage diagnosis or cancer deaths: no randomised trial of it has reported either outcome, and its published performance comes from case-control or single-arm studies. The only randomised result for any multi-cancer test, NHS-Galleri, did not show fewer stage III or IV cancers.
- What a result means: a positive result is a reason for further tests, not a diagnosis; a negative result does not rule cancer out and does not replace the screening tests that have been shown to work (bowel, breast, cervical and, where eligible, lung screening).
